City & Hackney Teaching Primary Care Trust (CHtPCT) invites interested voluntary sector organisations to bid for a local bereavement counselling service for people aged 18 to 50, who are either registered with a City & Hackney GP or resident of London Borough of Hackney or Corporation of London.

During autumn 2007, City & Hackney tPCT undertook a baseline review of local End of Life Care Services. This review involved a wide range of organisations involved in local end of life services; it highlighted an initial range of key areas that further work or investment needed to take place, during 2008/2009. One such service that needed to be developed was a local bereavement counselling service for 18 to 50 year olds.

The overarching aims and objectives of this service are to:

  • Deliver a Bereavement service to individuals and carers of individuals, aged 50 and under, living in the City and Hackney boroughs, who have been identified, either by themselves or by other health and social care professionals, as coping with the last phase of life;
  • Work with identified individuals and their families and carers in both the pre- and post- EOLC phase;
  • By means of this service, to enable a greater understanding locally of the issues facing individuals and their carers coping with the last phase of life and to feed this back by;
  • Contributing to and participating in strategic discussions with regard to the development of End of Life Care services;
  • Providing training for allied health professionals and others as identified on bereavement issues;
  • To deliver the service in a flexible manner which takes account of Service Users racial and cultural or religious background and respects individual dignity and privacy.
